Update visual design of the Manage Email Preferences Form

Right now, all Bloomerang "Manage Email Preferences" forms look the same. They're all blue and look very outdated. I know this because I am an email subscriber to other organizations that also use Bloomerang.

It would be great to have the ability to customize the search screen results. There are several fields depicted, which is great, but others such as constituency or class years would be helpful when two or more constituents have the same name.

ease of quickly identifying how many reoccurring donations a donor has (some donors have two or more concurrently, which gets confusing)
