Include 'Additional Donation' capability to Membership forms

We use the standard form to allow our constituents to buy/renew memberships. It would be helpful to also give them the ability to make an extra donation in addition to the membership fee. This is something that many of our constituents take advantage of when they pay via check. So we want ot be able to offer the same thing in our Bloomernag portal.
