Form questions at shift level

When there are multiple shifts in an opportunity, it would be great to be able to ask form questions at the shift level. For example, if only one shift will be providing food, being able to ask about allergens or dietary restrictions only to volunteers in that shift, or if a volunteer is attending multiple shifts, being able to ask how many kids they're bringing for each shift they sign up for.
