Add description text for appeals

Create a note field for appeals and funds to allow users to add a description of the appeal/fund. This would be incredibly helpful for organizations with dozens of appeals: to add a date range for an appeal, to understand what each appeal is used for, to explain why an appeal is no longer being used, etc.
